A Few Frequently Asked Questions


What do I need to bring to my first visit?
Please bring your driver’s license or picture ID, and your insurance card. If you do not have your insurance card yet, please contact your insurance company to get your policy number, group number, and filing address for claims, and plan type. Please also bring a list of all of your medications, and any over-the-counter supplements or vitamins you are taking. It is very important to bring this list to every visit and to be sure that you have the actual dosage and frequency you are taking them. 


How should I prepare for a physical?
Please be sure to not eat or drink anything after midnight the night prior to your appointment. You may have black coffee or water. This ensures that your cholesterol and diabetes screenings will be accurate. Please also avoid oils or lotions on your skin.


Do I have to go to another facility to get my blood drawn?
No, we can do your blood work right here in our office. We have an experienced phlebotomist who is here M-F.


Is gynecological care provided?
Yes, the “Well Woman Exam” is the office visit that you would schedule for your pap smear and mammogram order.


Are you accepting patients without insurance?
Yes, we do have a self-pay fee schedule based on what needs to be done at the visit.

We accept cash or credit cards only for self-pay patients. If you are receiving one of our rejuvenation and wellness procedures, we also offer CareCredit, which helps you pay for out-of-pocket expenses not covered by medical insurance by receiving special financing options that you can't get when using your Visa or MasterCard.


Do I have to pay my co-pay at the time of my visit?
Yes, the co-pay is due at the front desk prior to seeing the doctor. If you are uncertain of your insurance’s co-pay amount, we recommend you call your provider prior to your visit. Our office staff also is available to help you determine your co-pay.


Which insurance plans do you accept?
We take most major insurance plans, except Kaiser HMO. As with many physicians, we also accept Medicare. Please contact our office at (770) 495-9995.


Where are you located exactly?
We are conveniently located near the intersection of Peachtree Parkway/Hwy 141 and Abbotts Bridge Road (Hwy 120). Our address is: 6290 Abbotts Bridge Rd., Building 200, Suite 201, Johns Creek, GA 30097. Parking is available directly in front of the office, which is handicapped accessible.
